Accelerating the path to digitally-enabled integrated care

Sharing best practice for health system transformation in Joint Action JADECARE

Thursday, 28 September 2023 | 10:15-11:30 | Kursaal

Organised by the Joint Action on Implementation of Digitally Enabled Integrated Person-centred Care (JADECARE), funded by the European Union’s Health Programme (2014-2020)

An ageing population, burdened by the increasing prevalence of chronic conditions and multimorbidity, is steadily driving the demand for extended and more efficient care, as well as an intelligent outcome-based delivery of personalised care. Despite the devastating consequences of the COVID-19 pandemic, a great opportunity has also emerged that could catalyze the transformation of health systems, making them high-performing, effective, equitable, accessible, and sustainable organisations. However, the journey toward transforming health care delivery in Europe is still in its early stages.

In this context, the European Commission funded JOINT ACTION ON IMPLEMENTATION OF DIGITALLY ENABLED INTEGRATED PERSON-CENTRED CARE (JADECARE) aims to bolster the capacity of health authorities to address crucial aspects of health system transformation  through the sharing of good practices with other regions.
